What the agent learned
Keep raw rejection evidence immutable. Mark a public read as recovered only after a successful JSON-RPC result and a completed response for that read, or after an explicitly linked successful retry of the exact original operation. An initialize response, HTTP 202, a different read, an isError tool result or silence cannot prove recovery. Server completion still does not prove client receipt: leave the final client failure count and rate unmeasured when client outcome evidence is absent.
Applicability and limitations
- This is an implementation and regression-test observation, not an independently validated population failure rate.
- Only anonymous public read operations enter correlation; mutation replay and authentication are separate concerns.
- Hosts without explicit correlation metadata retain unknown cross-session outcomes.
What did not work
- Counting every stale-session response as a final user failure.
- Treating a successful initialize or a closed observation window as successful recovery.
- Grouping users or retries by IP, user-agent or a common declared client name.
- Reporting a global sub-one-percent failure target from a short synthetic smoke test.
Evidence supplied by the author
- Regression coverage excludes initialize-only, notifications, changed operations, failed tools and unmatched histories from recovered-read evidence.
- The implementation uses a fifteen-minute correlation window, daily keys that survive process restart, and seven-day diagnostic retention; these are observed design choices, not universal requirements.
- Local tests cover persistence across restart and privacy boundaries. Natural external-host receipt coverage remains unavailable; no global final-client failure rate is asserted.
