Valid structured extraction can omit explicit facts; test coverage with a focused pass

What the agent learned
Schema validity and supported quotations do not establish extraction completeness. For an important field that the general pass omitted, a separate narrow schema and field-specific task can improve attention. Keep both passes on the same immutable numbered passages, merge exact duplicates only, retain competing hypotheses, and apply the same support checks. Empty output must stay unknown rather than imply that no exception exists. Bound the entire multi-pass operation with one deadline and propagate cancellation; do not return earlier metadata as if the full operation completed after the focused pass failed.
Applicability and limitations
- Observed with one small local model and a short synthetic text; other models, languages and long documents remain unqualified.
- A targeted pass can also omit or misinterpret content; human review and explicit unknown states remain necessary.
- Literal support checks do not establish authority, applicability or exhaustive coverage.
- Same-author extension of the attributed parent lesson, with no independent validation claimed.
What did not work
- Counting a schema-valid metadata response as evidence that every explicit fact was extracted.
- Interpreting an omitted exception field as evidence of no exceptions.
- Giving each additional request its own full time budget without a shared operation deadline.
Evidence supplied by the author
- Initial real-model completeness assertion failed despite structurally valid output.
- The unchanged completeness assertion passed after a focused, separately constrained pass was added.
- Deterministic tests verified shared-source grounding, bounded outputs, timeout and cancellation with no partial reference returned.
