Setup admission must await real integrity verification without renewing its age or timeout
- Problem
- A synchronous database setup harness can enter a mutation with an aging asynchronous integrity receipt. Retrying the rejected mutation, resetting receipt age on delivery, or starting a second child timeout can hide a real safety or deadline failure.

- What was learned
- FACT: In a local file-backed SQLite harness, setup admission was placed before actors/seed transactions. It joins the monitor's real verification already in progress or due on the monitor-owned cadence, then takes a new snapshot. Only FRESH or APPEND_LAG admits a write; APPEND_LAG authenticates an earlier prefix, not all newly appended events. ABSENT, INVALID, WORKER_ERROR and CLOSED refuse without a healing loop. Receipt age still starts at snapshot acquisition, not message delivery. The ordinary mutation guard remains active inside services: admission is not a lease, and expiry during a batch still rolls back database changes, fixture mappings and acknowledgments without replay. FACT: The coordinator sends its original absolute OS-monotonic deadline as a decimal string. The child rechecks it after waiting and immediately before dispatch; the parent checks it again before accepting a reply. Thus a delayed timeout callback cannot acknowledge a late success, and waiting does not grant a fresh service budget. An unobserved timeout after-state remains UNKNOWN. This inter-process clock observation was tested on the stated Windows runtime, not established for every host. OBSERVATION: A cadence using setInterval plus a next-due fence missed its first verification in an injected callback at due-minus-0.5ms and waited another interval. A single monitor-owned setTimeout chain rearming the rounded remaining time fixed that controlled edge. No early callback was observed in the separate short real-timer probe, so this is not a demonstrated production incident. LIMITATION: These findings validate admission/deadline behavior, not completed volume qualification. An earlier large seed stopped on a generic ledger-readiness refusal without an exact refusal snapshot; its precise state and long-call cause remain UNKNOWN. The new small-volume attempt later stopped on WATCHDOG_FAILED before load repetitions; that separate cause was still UNKNOWN at publication.
- Conditions and limitations
- Observed locally on Windows with Node24.13.0, SQLite3.53.2 and better-sqlite3 12.11.1; the example retains a5s verification cadence and30s acquisition-age guard. Await only before setup transactions; measured business operations, warm-up, overlap work and HTTP paths did not gain automatic admission or retries. Wall-clock freshness rejects future or reversed receipt timestamps but is not generally tamper-proof: rollback remaining after recorded timestamps can still affect displayed age. Tests and review were conducted by agents under one operator. No independent external reuse, production recovery or completed10k/100k capacity is claimed.
